Warfarin
Warfarin is often referred to as a blood thinner, and is used to prevent blood clotting as it inhibits vitamin K; it is the most commonly prescribed anticoagulant.
Warfarin is taken as a tablet. Regular blood testing is required to monitor the dose of Warfarin, to ensure it is safe. A dose of Warfarin must be taken in the evening (up to midnight).
If a dose is missed you must inform the GP, update care records, and continue evening doses (no catch-up doses).
A white tablet of Warfarin is 0.5 milligrams
A brown tablet of Warfarin is 1 milligram
A blue tablet of Warfarin is 3 milligrams
A pink tablet of Warfarin is 5 milligrams
It is safe practice to attach the written, oral anticoagulant dosage supplied by the lab to the MAR chart."
